ONKOGINEKOLOGIK KASALLIKLARDA SITOKINLAR PROFILI VA YALLIG‘LANISH BIOMARKERLARINING DIAGNOSTIK HAMDA PROGNOSTIK AHAMIYATI: PROSPEKTIV KOHORT TADQIQOTI

Onkoginekologik o‘smalarda (bachadon bo‘yni, tuxumdon, bachadon tanasi saratoni) sitokinlar (IL-6, IL-8, TNF-α, IL-10, IL-1β) va yallig‘lanish biomarkerlari (CRP, fibrinogen, NLR) diagnostik-prognostik qiymatini baholash. Prospektiv tadqiqotda 180 bemor va 40 nazorat guruhi tekshirildi. Biomarkerlar ELISA va hs-CRP usulida aniqlanib, ROC, Kaplan-Meier va Cox modellari qo‘llanildi. Bemorlarda biomarkerlar nazoratdan ishonchli yuqori bo‘lib (p<0,001), maksimal qiymatlar tuxumdon saratonida (IL-6: 72,4±18,9 pg/ml; TNF-α: 44,3±11,7 pg/ml) qayd etildi. IL-6+TNF-α+CRP kombinatsiyasi eng yuqori diagnostik aniqlikni (AUC=0,947) ko‘rsatdi. Yuqori IL-6 (>32 pg/ml) va NLR (>2,8) ko‘rsatkichlari 3 yillik yashash darajasi pasayishi bilan bog‘liq (38,4% va 41,2%; p<0,001). Koks regressiyasida IL-6 (HR=2,43; 95% CI: 1,61–3,67), NLR (HR=1,89; 95% CI: 1,28–2,79) va o‘sma bosqichi (HR=3,17; 95% CI: 2,04–4,93) mustaqil prognoz omillari sifatida tasdiqlandi. IL-6 va NLR onkoginekologiyada bemorlarni to‘g‘ri tabaqalashtirish va davolashni optimallashtirishga xizmat qiluvchi muhim prognostik prediktorlardir.
